To adopt a dog from Rocky Mountain Collie and Sheltie Rescue (RMCSR) you will be required to sign a contract that requires compliance with all of the following terms and conditions. An RMCSR volunteer will go over the contract with you in person at the time of adoption.

ADOPTION CONTRACT
I hereby acknowledge receipt of the animal described on this Adoption Contract in return for which I agree to be bound by, and to comply with, all of the following terms and conditions of adoption.
1. I will provide proper and sufficient food, water, shelter, and kind treatment for the animal at all times.
2. I will have the animal examined annually by my veterinarian and recommended care done at my expense.
3. I will, if the animal is not already at the time of adoption, have the animal spayed or castrated as arranged by Rocky Mountain Collie and Sheltie Rescue by _____________ (date). In the event I fail to have the animal spayed or castrated within said time, I understand and agree that I will forfeit the adoption fee and I agree that I will, upon demand, immediately return the animal to Rocky Mountain Collie and Sheltie Rescue
4. I will provide an I. D. Tag with my personal contact information and keep it on the Collie or Sheltie at all times.
5. I will not sell, abandon or re-home this dog, and agree that if I no longer desire to retain the dog for its entire lifetime I will return it to Rocky Mountain Collie and Sheltie Rescue.
6. I agree that a representative of Rocky Mountain Collie and Sheltie Rescue may periodically wish to see the animal, my residence, and the living conditions which I provide for the animal, and I further agree to surrender the animal to a representative of the Rocky Mountain Collie and Sheltie Rescue if for any reason he/she considers the conditions provided by me for the animal unsuitable. I will keep Rocky Mountain Collie and Sheltie Rescue informed of any address change.
7. I will not permit my dog to run at large or to become a public nuisance. I also agree to immediately retrieve the dog at my expense from any public pound or animal shelter when notified that the animal is being held at any such location. In the event my dog becomes lost, I will make every reasonable effort and attempt to relocate and claim my dog and will notify Rocky Mountain Collie and Sheltie Rescue.
8. I agree that I will at no time assert any claim, charge, or demand of any kind against Rocky Mountain Collie and Sheltie Rescue for any expenses, which may have been incurred by me, including veterinary fees, in connection with my dog. I also agree that any illness or injury occurring after adoption will be my sole responsibility.
9. I hereby accept my dog as is, assume all risk of ownership of my dog, including the risk of injury or damage caused by the animal.
Rocky Mountain Collie and Sheltie Rescue may specifically enforce this contract through judicial proceedings. I hereby agree that in the event I breach this contract and Rocky Mountain Collie and Sheltie Rescue files suits to enforce this contract or to defend any claims under this contract, that I will pay any court costs and attorney’s fees incurred by Rocky Mountain Collie and Sheltie Rescue in connection herewith.
