ALL VOLUNTEERS:

By submitting this online application to Rocky Mountain Collie and Sheltie Rescue (RMCSR), I understand that it is my decision to foster any particular dog. I will not hold RMCSR, or any member thereof, responsible for any damage, injury, or harm caused, directly or indirectly, by any dog I decide to foster. I agree to keep identification (collar and ID tag) on the dog AT ALL TIMES.

I agree to be cautious when introducing a new foster dog to new people and/or dogs. I will always keep a foster dog on a leash when outside of my home or yard, and agree not to take the foster dog to any off-leash dog parks.

I agree to not to re-home or relocate the foster dog without explicit permission of RMCSR. If I locate a potential adoptive family on my own, I will direct them to RMCSR website and encourage them to fill out a adoption application.

I will contact RMCSR regarding any veterinary needs the foster dog may have, unless it is a life-threatening emergency. I will use veterinarians that are scheduled for or recommended to me by RMCSR. If I should choose to take the foster dog to my personal vet, I will not expect reimbursement without prior approval by an officer of RMCSR.

In the event that I am unable to continue as a foster, I agree to return immediately any dog in my care to RMCSR. Additionally, all records pertaining to foster dogs past and present shall be returned to RMCSR within 10 days thereafter. I further agree that all rescue records become the property of RMCSR.

In public dealings, I will remember that I am a representative of RMCSR, and shall make every effort to promote the mission of RMCSR. This includes not only answering questions accurately and politely, but also referring questions to more knowledgeable resources whenever necessary.

I have completed this application fully and accurately to the best of my ability and knowledge.
